Getting Ready for Your Visit
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

• Photo ID
• A list of current medications
• any and all x-ray films relating to the visit
• Insurance information and card
• information about past treatment and medication
• Past medical records that you think may be helpful

Don’t forget to bring your insurance information with you
In order to register and treat you promptly, we request that our patients bring their current insurance information with them for each visit.

Q: Can I fill out the medical forms prior to my appointment with COMSI?
A: Yes go to www.COMSI.mobi website, and click on form icon on a sidebar before you arrive or on your way to our offices. 
Q: What do I need to bring for my first visit?
A: First time patients must bring identification and insurance information. If under 18 a legal guardian must be present with you.
Q: Do I need to be early for my scheduled appointment time?
A: Yes 10 to 15 minutes
Q: What clothing should I wear on the day of my doctor’s visit?
A: Wear loose fitting clothing; the doctor must be able to examine the area without restriction.
Q: Do I need to bring a companion with me?
A: Yes, if you are having a procedure done, you may not be able to drive.
Q: What information should I bring for the doctor?
A: Bring your insurance card, a detailed list of medications, past medical history, doctor’s notes, x-rays, and/or MRI’s.
Q: I’ve had a change of insurance since my last visit. When should I notify the office?
A: Notify the office prior to your next schedule visit
Q: Am I responsible for a payment at the time of my visit?
A: Yes, you are responsible for co-pays and deductibles. If you have no insurance, payment is due in full at time of visit.
Q: It is my first visit; will I be seeing a doctor?
A: Yes, you will be seen by the doctor that you made the appointment with.
Q: When do I use ice on my injury?
A: Use ice 10-15 minutes to relieve pain and to keep the swelling down.
Q: When do I use heat on my injury?
A: You would only use heat 10-15 minutes where the injury is no longer acute. Heat helps to restore range of motion and decrease soft tissue stiffness.
Q: If have a fracture, will I be put in a cast?
A: Maybe, the doctor will make a decision on whether a cast or a splint is the best for you. Severe fractures may require surgery.
Q: What happens if I need an X-Ray or an MRI?
A: We have X-Ray and MRI onsite.
Q: Will I need to have follow up care for rehabilitation?
A: It is a good possibility that you may need restorative therapies.
